TRYOUT REGISTRATION
*** A player cannot practice or tryout without providing a completed and active physical form. ***
  1. Fill out the Contact Registration Form
  2. Print out and turn in Tates Creek High School Athletics Packet​
    1. (All of the following are REQUIRED to try out and participate in highschool athletics as mandated by KHSAA and FCPS.)
      1. KHSAA Consent & Physical Form
      2. UK Sports HIPAA Privacy Release Form
      3. TCHS Address Verification Form
      4. FCPS MRSA Policy
      5. Student Athlete Policy
      6. FCPS Middle School Playing Up Form
      7. FCPS Transportation Volunteer Driver Checklist
  • Requets:
    • Do not staple, paper clip is fine
    • Single Side Print
    • 3 copies of completed KHSAA Consent & Physical Form (keep the original) 
 
  • New players
    • Must bring to first practice that you attend
  • Returning players
    • Covered by last year’s form for early practices
    • Must provide new form when old one expires
  • Required a physical by a Doctor, Urgent Care, Little Clinic, etc.

6/3 (M) 8:30-10:30am: Kick-Arounds*
6/10(M) 8:30-10:30am: Kick-Arounds*
6/17(M) 8:30-10:30am: Kick-Arounds*
* Need to have an active Physical submitted to coach.

6/25-7/9: KHSAA DEAD PERIOD (No communications between coaches and students)
7/14 (Su) 5:00pm: Packet Turn-In
7/15 (M) 8:00am - 10:00am: Tryouts*
7/16 (Tu) 8:00am - 10:00am: Tryouts*
7/16 (Tu) by 6:00pm: Teams Announced
7/17-7/19 7:00am - 9:00am Practice
7/20 (Sa) 6:00pm Team Meeting and Pot Luck
7/22-7/26 7:00am - 9:00am Practice
7/29-8/2 7:00am - 9:00am Practice

8/6 (Tu) 6:00pm - 9:00pm Scrimmage @ Madison Central
8/8 (Th) 6:00pm - 9:00pm Scrimmage @ South Oldham
